OWHA VACCINATION POLICY IMPLEMENTATION AT COLGHA

The OWHA has invoked a vaccination policy effective immediately that applies to all Collingwood Girls Hockey Association members.  The OWHA requires that all affected persons be Fully Vaccinated against COVID-19 as a condition of participation in any game, on-ice practice, off-ice training, or tournament.  At present, this policy does not apply to players born after 2009 but may be subject to change at any time.

Affected persons of this policy include:

  • Vaccine-Eligible Players (born in 2009 or earlier)
  • Team Officials
  • Game off-ice/on-ice officials
  • All COLGHA Executive & Association Volunteers
  • Instructors, supervisors, development providers & others involved on or off-ice in any activity

Requirement

Each affected person is required to be Fully Vaccinated (including the 14-day period after receiving their completed dose) by November 1, 2021 or earlier.

Each affected person must present evidence demonstrating they have received the completed series of an Accepted COVID-19 Vaccine by October 17, 2021 or earlier.

  • Teams trainers will verify evidence for all team players and staff as applicable and confirm all members by name satisfy the requirement to the Head Trainer by Oct. 17, 2021 (trainer@collingwoodgirlshockey.com)
  • The Referee in Chief will verify evidence for all on-ice and off-ice officials and confirm all members by name satisfy the requirement to the Head Trainer by Oct. 17, 2021
  • All other volunteers, not working as part of teams/officials are to demonstrate evidence to the Head Trainer by Oct. 17, 2021

Acceptable evidence and documents include:

  • Digital dose administration receipt
  • Medical records signed by a licensed health care provider on that provider’s letterhead indicating vaccine name and date(s) of administration
  • Government verification/language (eg: Government issued vaccine passport, certificate, card, or electronic record) (can be obtained at ontariohealth.ca)

Medical Exemptions

Any affected person seeking medical exemption should visit section 6 of the OWHA policy to start the request https://www.owha.on.ca/content/updates-amp-bulletins

 

Any completed medical exemption form should be presented to Team Trainers, Chief Referee or to the Head Trainer for verification.

 

Unvaccinated Affected Persons

After October 17, 2021, any Affected Person who is not Fully Vaccinated (excluding the 14-day period after receiving their completed dose) or who has not been granted an accommodation in accordance with the OWHA Policy, will no longer be eligible to participate in any Sanctioned Activities until such time as they have presented satisfactory evidence that they have been Fully Vaccinated (including the 14-day period after receiving their completed dose) or have been granted an accommodation.
